Active-slot items may fail; returning to the main menu is a workaround, not a fix.

Workaround status matters
A workaround is not the same as a fix. Restarting to the main menu may restore an Active item, and Recover Save may help a loading crash, but neither proves the underlying bug is resolved. Recheck the patch tracker after every update.
Report without losing useful detail
The official template asks for bug type, exact location, build version, reproducibility, platform and PC specifications where relevant. Add screenshots or video only when they show the state clearly.
